With reference to the call of my representative, Miss Bailey, at the Asylum on Monday last, I have pleasure in sending you herewith copy of the original from of application which was filled in on behalf of the above girl on her first being received under the Society's care, i.e. in 1892. I trust the information contained therein may be of some service to you, but, should you like any further particulars on any point, I hope you will not hesitate to let me know.

I am very sorry to hear such a sad account of the poor girl, and perhaps you will kindly let me know how she continues after a time?
