I hereby certify that A. requires An Artificial Leg & Foot and that the cost of the same will be equal to 30 Letters of Recommendation for which the Patient is hereby authorised to make application to the Subscribers; satisfactory information having been registered as to the Patient's circumstances and need of assistance.

On satisfactory proof being given of the Patient's inability to obtain all the Letters, the Secretary will be prepared to recommend this as a SPECIAL CASE to the Committee.
