I have duly received your application on behalf of the above child, but inasmuch as I note that he is a cripple, I must trouble you to be good enough to have the enclosed medical certificate forms filled in on his behalf, by separate Doctors, and returned to me, when I shall be pleased to submit the case to the next meeting of the Cripples' Committee on the 13th. inst
